Patient Referrals (For Medical Institutions)

To reduce waiting times for referred patients, we recommend making an appointment in advance. Please follow the procedures below.

Requesting a Consultation

Please refer to the Outpatient Physician Schedule.
Please complete the Consultation Request Form.
Please send it by fax to the Community Liaison Office (FAX: 06-6881-6530).
A staff member will contact you by phone to confirm and arrange a scheduled date.
An appointment confirmation notice will be sent to you by fax.

※ Endoscopy	Upper and lower endoscopies are performed following a consultation.

Please fax the application form to 06-6881-6530. Questionnaires do not need to be faxed — please bring them on the day of the examination.

Regarding Results

Our hospital does not provide result explanations directly to patients. If you would like a written report of findings, please let us know at the time of booking.

Other Examinations

Bone density measurement (no appointment necessary — walk-ins accepted on weekdays 14:00–16:00)

What Patients Should Bring

Please bring the appointment confirmation notice, referral letter, health insurance card, and proceed to General Reception ②. For MRI and contrast CT, please also bring the completed questionnaire.
