Ubie AI Medical Questionnaire

Ubie is an AI-powered medical questionnaire used at Kano General Hospital. Before your visit, you can answer questions about your symptoms on a computer, tablet, or smartphone.

Our hospital has used this system since March 2020. Based on the symptoms and information entered by the patient, the system automatically selects follow-up questions. As a result, hospital staff can check your condition more smoothly before consultation.

You may complete the questionnaire at home or anywhere else before coming to the hospital. However, completing it does not create a reservation. Please check the notes below before using the service.

How to Complete the Ubie Questionnaire

First, select the department that matches your visit. Then, answer the questions shown on the screen. The questions may change depending on your answers.

Next, follow the instructions until the questionnaire is complete. If you are not sure how to answer a question, please choose the option that best matches your current condition.

Finally, please come to the hospital as usual. Staff may check your questionnaire information when needed.

Who Should Use It

This questionnaire is intended for patients who need to share symptom information before consultation. For example, it may be useful if you are visiting for the first time or if your symptoms have changed.
