Preparing for Hospitalization

Admission Procedures

1. For Patients with a Scheduled Admission

If you are unable to arrive on the designated date and time for your scheduled admission, please be sure to contact us in advance. If we do not hear from you, your admission may be cancelled.

2. On the Day of Admission

Please prepare the following items and complete your admission procedures at the Admission Desk (Windows (6) and (7)). (Z Patients admitted on an emergency basis are also asked to complete the procedures as soon as possible at a later date.)

  1. Patient registration card
  2. My Number health insurance card (Please note that failure to present this card may result in charges being billed as self-pay.)
    As we will need to verify this each month during your stay, please present it at the Admission Desk (Windows (6) and (7)).
  3. Patients receiving public assistance, those involved in traffic accidents (automobile or motorcycle), or those covered by workers’ compensation insurance should inform the front desk accordingly.
  4. Hospitalization pledge and room rate difference consent form
  5. Personal seal (hanko)
  6. Admission deposit of ¥70,000 (please refer to item 1 under “About Hospitalization Costs” below)
  7. Any medications you are currently taking

3. Items to Bring Upon Admission

Toothbrush set, cup, tissues, underwear, slippers, non-slip shoes, towels, bath towels, hangers (if needed)

  • In the case of emergency admission, please check with the nurses’ station.
  • The above items can also be rented through our contracted vendor.
