Individualized Rehabilitation Program Tailored Just for You
A team of rehabilitation specialists—including Physical Therapists (PTs), Occupational Therapists (OTs), and Speech-Language-Hearing Therapists (STs)—collaborate to create an individualized program for each patient.
From the acute phase through the recovery phase, we provide rehabilitation on a 365-day basis, supporting not only functional recovery but also with a clear view toward life after discharge.
Additionally, through home-visit rehabilitation after discharge, we have established a system to ensure seamless, uninterrupted rehabilitation can continue.

Multidisciplinary Team Rehabilitation
Centered on the rehabilitation physician, a multidisciplinary team including nurses, registered dietitians, and medical social workers provides comprehensive support for every patient.
Family Caregiving Instruction and Pre-Discharge Support
With an eye toward life after discharge, we provide caregiving instruction for family members, pre-discharge home visit guidance, and community outing practice to support a smooth and confident transition back to home life.
Community Collaboration
We actively participate in community care conferences and local meetings, working in close collaboration with community physicians, nurses, and care managers.
Physical Therapy
Combining therapeutic exercise and physical modalities, physical therapy aims to restore basic motor function—including rolling over, sitting up, standing, and walking—to support independence and social reintegration.

Based on thorough assessment, we safely and effectively implement range of motion exercises, muscle strengthening exercises, and more. - Movement Training and Instruction
We select and fabricate appropriate assistive devices and lower limb orthoses, and provide movement training suited to each patient’s living environment and lifestyle. - Physical Modalities Tailored to Individual Symptoms
We utilize thermotherapy, ultrasound therapy, functional electrical stimulation (FES), and other modalities to achieve pain relief and functional improvement.
Occupational Therapy
Occupational therapy supports not only activities of daily living but also the reconstruction of a lifestyle and social roles that are uniquely one’s own.
We provide holistic support—addressing both mind and body—to help patients resume the daily activities and hobbies lost due to stroke, upper extremity trauma, and other conditions.
Speech-Language-Hearing Therapy
We conduct assessment and training for aphasia, dysarthria, higher brain dysfunction, and dysphagia.
In collaboration with physicians, nurses, and registered dietitians, we also perform specialized evaluations including videofluoroscopic swallowing studies (VFSS).
Cardiac Rehabilitation
We provide outpatient cardiac rehabilitation for patients recovering from myocardial infarction, heart failure, cardiac surgery, and related conditions.
By carefully monitoring cardiac status and prioritizing patient safety, we deliver therapeutic exercise programs aimed at restoring physical fitness, preventing disease recurrence, and improving quality of daily life.
Under physician direction and led by physical therapists, we monitor heart rate, blood pressure, and symptoms to provide rehabilitation tailored to each patient’s condition and lifestyle background.
Beyond exercise therapy, we also support patients in managing daily activity levels appropriately and building sustainable exercise habits, ensuring they can safely and confidently continue living at home.
Home-Visit Rehabilitation
Even after discharge, we provide seamless, uninterrupted rehabilitation to prevent functional decline and support patients in continuing to live safely in their familiar community.
We offer comprehensive life support, including exercise guidance and advice on home environment modifications.
