Patient Referrals (For Medical Institutions)<\/h3>\n

To reduce waiting times for referred patients, we recommend making an appointment in advance. Please follow the procedures below.<\/p>\n

Requesting a Consultation<\/h4>\n

  2. Please complete the Consultation Request Form<\/a>.<\/li>\n
  3. Please send it by fax to the Community Liaison Office (FAX: 06-6881-6530).<\/li>\n
  4. A staff member will contact you by phone to confirm and arrange a scheduled date.<\/li>\n
  5. An appointment confirmation notice will be sent to you by fax.<\/li>\n<\/ol>\n<\/div>\n

    Please fax the application form to 06-6881-6530. Questionnaires do not need to be faxed \u2014 please bring them on the day of the examination.<\/p>\n

    Regarding Results<\/h4>\n

    Our hospital does not provide result explanations directly to patients. If you would like a written report of findings, please let us know at the time of booking.<\/p>\n
