(\uff3a Patients admitted on an emergency basis are also asked to complete the procedures as soon as possible at a later date.)<\/p>\n<p><!-- [\u4fee\u6b63] ol\/li \u306b\u30af\u30e9\u30b9\u4ed8\u4e0e\uff08\u30eb\u30fc\u30eb6\u6e96\u62e0\u3002\u756a\u53f7\u306e\u610f\u5473\u3092\u4fdd\u6301\u3059\u308b\u305f\u3081 ol \u306e\u307e\u307e\u4f7f\u7528\uff09 --><\/p>\n<ol class=\"KoteiPage_InfoList\">\n<li class=\"KoteiPage_InfoList_Item\">Patient registration card<\/li>\n<li class=\"KoteiPage_InfoList_Item\">My Number health insurance card (Please note that failure to present this card may result in charges being billed as self-pay.)<br \/>\nAs we will need to verify this each month during your stay, please present it at the Admission Desk (Windows (6) and (7)).<\/li>\n<li class=\"KoteiPage_InfoList_Item\">Patients receiving public assistance, those involved in traffic accidents (automobile or motorcycle), or those covered by workers\u2019 compensation insurance should inform the front desk accordingly.<\/li>\n<li class=\"KoteiPage_InfoList_Item\">Hospitalization pledge and room rate difference consent form<\/li>\n<li class=\"KoteiPage_InfoList_Item\">Personal seal (hanko)<\/li>\n<li class=\"KoteiPage_InfoList_Item\">Admission deposit of \u00a570,000 (please refer to item 1 under \u201cAbout Hospitalization Costs\u201d below)<\/li>\n<li class=\"KoteiPage_InfoList_Item\">Any medications you are currently taking<\/li>\n<\/ol>\n<p><!-- [\u4fee\u6b63] h5 \u2192 h4 \u306b\u5909\u63db\u3001KoteiPage_SubTitle KoteiPage_SubTitle--sub \u3092\u4ed8\u4e0e\uff08\u30eb\u30fc\u30eb4\uff09 --><\/p>\n<h4 class=\"KoteiPage_SubTitle KoteiPage_SubTitle--sub\">3.
